INSURANCE & BILLING INFORMATION: We will gladly submit your bill to an insurance company, however it is very important that you provide with current insurance information. Proper information being: a copy of your insurance ID card, or, if the insurance is new, a copy of your application for coverage including the group policy number. Patients who belong to health maintenance organizations must change their primary care physicin to this office prior to being seen. You may be responsible for a portion or all of the charges incurred, if we do not receive the proper insurance information. While we make every attempt to stay as up to date with covered benefits for the various insurance companies, we ask that you contact the customer service department of your health insurance if you have questions about covered services. They will have the most accurate information regarding your individual policy. We will submit claims to a motor vehicle/worker?s compensation insurance company, however, if you have not opened a claim through your automobile insurance/employer, you may be responsible for a portion or all of your bill.
